All physicians practicing medicine in the United States are required to pass a series of standardized board exams known as the United States Medical Licensing Exams (USMLE), which consists of the Step 1, Step 2 Clinical Knowledge (CK), and Step 3 exams. Prior to January 26 of this year, all three test grades were reported as a numeric score in addition to the designation pass or fail. Since January 26th, the USMLE and its sponsors, the National Board of Medical Examiners (NBME) and the Federation of State Medical Boards (FSMB), have decided to forego a numeric grade for the Step 1 exam, making the score designated only as pass or fail.

The reason for the change? According to Kevin Jubbal, founder of Med School Insiders, the change occurred to Strengthen the well-being of medical students and decrease the stress and anxiety of students surrounding the exam. According to the 2018 National Resident Matching Program’s Program Director Survey, the USMLE Step 1 score was the number one factor used when deciding which candidate to offer an interview for residency training (post-graduate training that occurs for each medical specialty immediately after graduating medical school). This test, often taken between the second and third years of medical school, aims to confirm minimal competency for licensure by testing fundamentals of the basic clinical sciences that are taught in the pre-clinical years of medical school. It is of no surprise that many medical students would often obsess over getting the highest score possible in order to join the career of their choice after medical school graduation.

Has the change to pass/fail of the USMLE Step 1 done what it was intended to do- namely decrease stress and anxiety in medical students? According to the 2021 National Resident Matching Program’s Program Director Survey, 94% of residency programs require a numeric score on the USMLE Step 2 CK test before offering interviews to candidates. According to another study, many residency programs now consider the USMLE Step 2 CK as the primary factor when considering which medical students to offer interviews for residency training. The stress and anxiety many medical students feel has simply shifted from the Step 1 test to the Step 2 CK exam. In other words, the USMLE Step 2 CK test is the new Step 1, when considering medical student well-being and mental health.

According to a study in JAMA Network Open, 50% of medical students are experiencing burnout. Although there has been much attention given to the crisis surrounding the dramatic rise of physician burnout since the Covid-19 pandemic, much less focus has been given to burnout in medical students, the future caregivers and first responders that will care for you and your loved ones. How can we expect future physicians to master clinical medicine and care for the ill when they cannot even care for themselves?

Equally troubling to the detrimental effects of well-being are the racial and ethnic disparities the USMLE brings to underrepresented medical student candidates for residency. According to data from an article in Academic Medicine, Black and Latino medical students are more likely to score lower and/or fail all three USMLE exams when compared to White students. The reason- downstream effect of decades of systemic racism whereby they are provided fewer resources and opportunities to succeed and excel in academics. Consequently, it is much more difficult for underrepresented minorities to receive interview offers in the specialty of their choice when graduating medical school when compared to White students.

The USMLE, NBME, and FSMB have a real opportunity to promote medical student and physician well-being as well as racial equity by addressing scoring on physician licensing exams. Just as they made the Step 1 pass/fail, they must make all USMLE exams pass/fail. This would undoubtedly reduce the enormous stress and anxiety medical students face during their arduous years in school. Furthermore, underrepresented students will perhaps have a fairer shot at pursuing their dream specialties after graduating. Standardized tests are merely one metric and measure for success. Eliminating numeric scores on the USMLE will force residency programs to make a more holistic review of prospective candidates.

Some medical schools are already doing this for medical school admissions. At the University of Maryland School of Medicine, the admissions committee performs holistic screening deemphasizing standardized tests scores, offers unconscious bias training for interviewers, and blinds interviewers to standardized scores; to name a few initiatives. Residency programs throughout the country should follow suit when considering future trainees in their programs. The USMLE, NBME, and FSMB as well as the hundreds of medical training programs throughout the country must start taking care of medical students, who we will all rely upon to take care of us.

Contracting is what happens before you initiate work with a client on a specific engagement. Contracting is not only about signing the legal agreement, though this undoubtedly is a significant part. It is also an opportunity to better understand context, manage expectations and seek alignment on the targeted outcome.

See it as the prologue in a drama before the plot plays out. If we start a project without aligning on the value to be delivered, we would only be running blindfolded, hoping, but never sure, that what we deliver will be of value to the client.

Contracting is also important to manage the expectations of the client. I have often experienced clients signing up a coach and expecting them to deliver a transformed coachee within a few months of an engagement. People who understand coaching know that coaching is mostly about behavioral change, which takes time. Your attempt as a coach is to leave the coachee at a point where they have accepted the need to change, are more aware and have started on their change journey with solid footing and little chance of skidding back.

Most coaching or team coaching engagements have multiple parties to contract with. You have a sponsor (organization or leader paying for the engagement) and the coachee or the team you will coach. Unless there is a "single mind" on the outcome, it is likely that one of the parties may not be satisfied with the outcome. Contracting is an opportunity to drive that alignment and set yourself up for success.

So, what are some of the questions that can help you get more from the contracting stage?

To The Sponsor

• What does the organization expect to achieve at the end of this engagement? The client's response helps you understand their expectations and correct them if entirely out of place.

• How would the organization know that the engagement has made a difference? This can help to better understand the assessment process and whether it is designed well enough to measure the impact you desire.

• What feedback has the coachee been given so far on their areas for improvement? Don't be surprised if the superior has not had any feedback sessions with the coachee and still expects the coachee to know what to work on. We may have to explain that the coach's job is to inspire the coachee to act on the feedback and not to supply feedback.

• What are the stakeholders willing to do to help with the change process? The stakeholders' (superior, HR manager or commissioners of the team) job does not end with enlisting a coach. The coachee needs their support as they watch the coachee work on the outcome. If stakeholder meetings are also part of the process, this is the time to clarify that these are best had with the coachee present to avoid any breach of confidentiality.

To The Coachee

Even if the organization is the sponsor, it has been my practice not to sign up a new coachee unless I have a chemistry session with them. I use this opportunity to ask the following questions to set the expectations right and assess their coachability.

• What do you expect to achieve at the end of this engagement? As with the client, it is equally important, if not more, to set the right expectations with the coachee.

• What are you willing to sacrifice to achieve the outcome? This helps you understand how meaningful this engagement is for the coachee. Let them know that besides each coaching session, the entire journey will need their time, energy and commitment.

• What made you start with this coaching engagement at this moment? Understand their motivation behind starting with this engagement and why it is important enough to deserve their focus and attention at this time.

• Why do you believe I am the right person to make this happen? Don't be surprised if you still only hear that they want your advice and the benefit of your experience. Take this as another opportunity to clarify the role of a coach.

Despite what most people think, contracting does not necessarily end with signing up for an engagement. In one of his sessions, Peter Hawkins introduced us to the acronym ABC, which stands for "always be contracting."

We contract in every session when we ask, "What is it that you would like us to explore today?" or "What do you expect to receive at the end of this session?" We also re-contract when something new emerges during the session. We may think we know what the client wants to discuss, but it is not what we think that matters but what the coach wants. The best way is a quick check-in question, "Before we take the discussion forward, tell me how we are progressing so far," or "How are we doing so far, in the context of the outcome we are working toward?"

A well-executed contracting phase is half the battle won, so let's invest enough in it—beyond just getting the client to sign on the dotted line.

If a class meets an equal number of times in each sequence, the examination should be scheduled according to the sequence which shows an earlier date or time in the examination schedule.

i.e., for MTWF or MWTHF courses, refer to the MWF examination time. For MTWTH of MTTHF courses, find both the MWF test time and the TT test time—your test is scheduled for whichever date/time is earlier.

All sections of Accounting 203 and 204 have a common test on Saturday, December 10, 9:00 a.m. All sections of Mathematics 171, 172, and 271 have a common test on Saturday, December 10, 9:00 a.m.

Industrial building sector construction, while healthy, might also be flattening

For all the hoopla about the ecommerce boom and “last mile” order fulfillment driving demand for more warehouse and manufacturing space, construction of industrial buildings actually declined over the past five years, albeit marginally by 2.1% to $27.3 billion in 2022, according to estimates by IBIS World. Still, construction in this sector remains buzzy.

Today, our bar examinees will be taking part in the fourth and last day of the 2022 Philippine bar examinations chaired by Supreme Court Justice Alfredo Benjamin Caguioa.

Just like last year, there are regional testing venues and the mode of taking the exams is digitized – using the computer-based Examplify software. So bad handwritings are no longer a problem.

The bar test covers the usual subjects – Political and International Law, Criminal Law, Labor Law and Social Legislation, Civil Law, Mercantile Law, Remedial Law, and Legal Ethics – but removed Taxation Law as a separate bar subject, and adopts a three-examiner per subject policy.

The Philippine bar examinations is without doubt the most difficult licensure examination in the country. The fact that the exams are in digital form did not diminish its difficulty – the questions still required of the examinees a recollection of what they learned in four or five years in law school, the ability to identify and analyze what is being asked, and applying the rule of law and their analysis to answer the question.

If you want to become a lawyer and carry that “Atty” as part of your professional name, then there is only way, and that is to pass the bar exams.

But of course passing the bar is no certain of success in the legal profession, just like in any other profession which requires a licensure test.

It likewise does not determine what kind of lawyer they will become.

As pointed out by the National Union of People’s Lawyers, the bar exam, “is merely a door at the very end of a very long and arduous passage that once opened, permits you into a expansive hall filled with opportunities and responsibilities.”

NUPL president lawyer Ephraim Cortez pointed out that the bar exams is only a test like most others and that prestigious though it may be, this test will not define one either as an individual or as a lawyer.

In the past, the bar exams also served as a competition among the different law schools for the top 10 highest grades. But last year, the top 10 grades were not announced, but instead, recognitions were given to the exemplary passers or those who scored 85 to 90 percent and the excellent performers or those who got ratings of over 90 percent.

Recognition was also given to the schools with the most number of excellent and exemplary passers.

This was because during the previous bar examination with Justice Marvic Leonen as the bar chairman, the Supreme Court decided to be more inclusive by drawing the focus from how every examinee performed competitively against each other but instead highlighting performance of law schools.

We do not know yet how it will be done under the Caguioa-led bar exam.

But one thing was made sure – it will not be as easy as the previous 2020-2021 bar, which was taken under the most difficult conditions due to the pandemic.

The 2020-2021 bar exam’s passing rate of 72.28 percent, which was one of the highest in the bar history (the highest was the 1954 bar with 75.17 percent), will also probably never happen again. A total of 8,241 examinees out of 11,402 takers passed the 2020-2021 exams held earlier each year. This year, around 9,100 took the exams. No bar exams were held in 2020 and 2021 due to the pandemic. The test which was originally scheduled in November 2021 had to be postponed to February 2022 due to increasing number of COVID-19 cases back then.

The November 2022 bar examinees were also required go on quarantine before and during the bar exams to avoid getting infected and had to be tested negative against COVID-19.

Moving forward, the Supreme Court has said that future bar examinations will be conducted via digital means to be in tune with technological advancements. And they will remain regionalized or localized unlike before when there was only one testing site and examinees from Visayas and Mindanao had to stay in Manila for at least a month for the exams which were held on four consecutive Sundays.

The bar examinations in the Philippines is still evolving to make sure that the examinees are only asked basic questions which entry-level lawyers should know how to answer. What is also being taught in law school is also being reviewed to make sure that it remains relevant.

And so for the thousands of students who have reached the end of their long journey, good luck.

Killexams : Road to success as contractor awarded National Highways projects

A tier one contractor has been awarded four projects worth a combined value of £20m by the National Highways as part of its office and depot upgrades.

Morgan Sindall Construction was selected to complete the works which will take place at four sites within Area 9 which takes in the areas of Gloucester, Shropshire, Staffordshire, Warwickshire, West Midlands, Worcester and Herefordshire.

The development is part of the National Highways’ plans to Strengthen its depots throughout England. It will be the first time they adopt a ‘hub and spoke’ model, which will see them build or upgrade satellite offices at their roadside depot locations.

Work has already started at Warndon, which involves the refurbishment of the existing office area and provision of a refurbished car park area. Morgan Sindall Construction will also begin work this month at Bescot, Doxey and Strensham depots.

At Bescot, a new two-storey office building will be provided with associated external works, including the removal of the existing buildings from site. Doxey also involves the provision of a new build two-storey office building with associated external works. Finally, Strensham includes a new build garage and salt barns with the refurbishment of the existing office and amenity space and the removal of the existing salt barn.

The new buildings will serve a multiple purpose, housing the local operational teams, non-operational personnel and visitors. To enable a flexible and agile performance, the design of the ground floor focused on providing the operational staff with all necessary amenities to function effectively when in these buildings. A shared canteen will be installed to promote staff interaction and further integration and ensure there is knowledge of what each team is doing.

The contractor will work in collaboration with project manager Gleeds and architect Harris Partners as well as National Highways’ maintenance and response partner, Colas.

National Highways Head of Service Delivery, Colin Lowther, said: “Once the work is completed, the look and feel of the depots will change to appear more like a modern National Highways workplace enabling operational and non-operational staff as well as contractors to work more closely alongside each other and encouraging interaction and collaboration between the teams.

“It was important to us to not only Strengthen the facilities for our staff and offer a flexible space but also to Strengthen safety which is always our number one priority. Segregating vehicle movements between operational and office staff will help keep people safe at the depots.

“We are pleased to be working with Morgan Sindall Construction and our partners on this project and welcome, in particular, the commitment to reducing carbon where possible in carrying out the work.”

Richard Fielding, area director at Morgan Sindall Construction, said: “We’re very proud to be working with National Highways to Strengthen the working conditions of people who carry out vitally important work.

Most of the projects will remain operational during construction so we will be working collaboratively with our partners to ensure we deliver the upgrades without disruption. Whilst all the projects will be different, we have created a component lead depot blueprint in order to streamline the process and replicate their success.“ Each of the four projects have been procured through SCAPE, one of the UK’s leading public sector procurement authorities.

SCAPE’s group chief executive, Mark Robinson, said: “We are delighted to be working with our partner, Morgan Sindall Construction helping to accelerate the project forward and deliver much-needed facilities for National Highways. Once complete, these four projects will be excellent examples of how contractors can collaborate effectively with the client and partners to reduce their carbon footprint and meet net zero ambitions, while also delivering exceptional social value within the community.”

As part of its Net Zero goals, Morgan Sindall Construction has run its Carbonica tool to reduce carbon where possible. Carbon saving initiatives in the build include LED lighting, solar panels, air source heat pumps, PIR zone control sensors and highly efficient M&E packages.

As week as engaging with a high proportion of local labour and build on its commitment to the community, Morgan Sindall Construction will also work carefully to ensure disruption is kept to a minimum for local residents, with special attention given to works to ensure they are delivered respectfully and sympathetically.
